Vildagliptin (Galvus)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Vildagliptin

What it is

Vildagliptin is a DPP-4 inhibitor: it blocks the enzyme that breaks down incretins, gut hormones such as GLP-1 that tell the pancreas to release insulin after a meal. It is used for type 2 diabetes, alone or with metformin (Eucreas, Galvumet), in Europe, Asia and elsewhere but not in the US. It is sold as Galvus.

How genes come into it

Vildagliptin is cleared mainly by hydrolysis, largely in the kidneys, and not by CYP enzymes. Studies have looked at genes of the incretin system (GLP1R, DPP4, GIPR) and diabetes genes such as WFS1, KCNJ11 and KCNQ1 for how much blood sugar fell. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

GLP1R rs6923761: Level 3. Efficacy: Diabetes Mellitus, Type 2. 1.

Response (25 findings)

Studies of how well it worked in people carrying a variant.

GLP1R rs3765467: Genotypes AA + AG is associated with increased clinical benefit to gemigliptin, linagliptin, saxagliptin, sitagliptin or vildagliptin in people with Diabetes Mellitus, Type 2 as compared to genotype GG.

GLP1R rs6923761: Genotype AA is associated with decreased clinical benefit to sitagliptin or vildagliptin in people with Diabetes Mellitus, Type 2 as compared to genotypes AG + GG.

GLP1R rs6923761: Genotype AA is associated with decreased response to sitagliptin or vildagliptin in people with Diabetes Mellitus, Type 2 as compared to genotype GG.

KCNJ11 rs2285676: Genotype GG is associated with increased clinical benefit to linagliptin, sitagliptin or vildagliptin in people with Diabetes Mellitus, Type 2 as compared to genotypes AA + AG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
