Alogliptin (Nesina)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Alogliptin

What it is

Alogliptin is a DPP-4 inhibitor: it blocks the enzyme that breaks down incretins, gut hormones such as GLP-1 that tell the pancreas to release insulin after a meal. It is used for type 2 diabetes, alone or combined with metformin or pioglitazone. It is sold as Nesina and Vipidia.

How genes come into it

Alogliptin is cleared mostly unchanged by the kidneys. The few gene studies listing it, from Japan, looked at PNPLA3 and IL6 for how much blood sugar fell. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Response (3 findings)

Studies of how well it worked in people carrying a variant.

PNPLA3 rs738409: Genotypes CG + GG is associated with increased clinical benefit to alogliptin in people with Diabetes Mellitus, Type 2, Fatty liver disease and Body Weight Changes as compared to genotype CC.

rs2097677: Genotypes AA + AG is associated with increased response to alogliptin, linagliptin, sitagliptin, teneligliptin or vildagliptin in people with Diabetes Mellitus, Type 2 as compared to genotype GG.

IL6 rs1800796: Genotypes CG + GG is associated with increased response to alogliptin, linagliptin, sitagliptin, teneligliptin or vildagliptin in people with Diabetes Mellitus, Type 2 as compared to genotype CC.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
