Sitagliptin (Januvia)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Sitagliptin

What it is

Sitagliptin is a DPP-4 inhibitor: it blocks the enzyme that breaks down incretins, gut hormones such as GLP-1 that tell the pancreas to release insulin after a meal. More incretin means more insulin when blood sugar is high and less glucagon. It is used for type 2 diabetes, alone or with metformin (Janumet). It is sold as Januvia.

How genes come into it

Sitagliptin is cleared mostly unchanged by the kidneys, so drug-processing genes play little part. Studies have looked at genes of the incretin system, such as GLP1R (the GLP-1 receptor), DPP4 and GIPR, and at diabetes genes such as KCNJ11, KCNQ1 and WFS1, for how much blood sugar fell. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

GLP1R rs6923761: Level 3. Efficacy: Diabetes Mellitus, Type 2. 1.

Response (34 findings)

Studies of how well it worked in people carrying a variant.

CYP2C9 rs1799853: Genotype TT is associated with decreased response to sitagliptin in people with Diabetes Mellitus, Type 2.

DPP4 rs2909451: Genotype TT is associated with decreased response to sitagliptin in people with Diabetes Mellitus, Type 2.

DPP4 rs4664443: Genotype GG is associated with decreased response to sitagliptin in people with Diabetes Mellitus, Type 2.

GLP1R rs3765467: Genotype AG is associated with increased response to sitagliptin in people with Diabetes Mellitus, Type 2.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
