Tropisetron (Navoban)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Tropisetron

What it is

Tropisetron is an anti-sickness medicine of the 5-HT3 antagonist group: it blocks serotonin receptors in the gut and in the brain's vomiting centre. It is used to prevent nausea and vomiting after chemotherapy and surgery. It is sold as Navoban, among other names, in Europe and other regions but not in the US.

How genes come into it

Tropisetron is broken down mainly by the liver enzyme CYP2D6. People with extra working CYP2D6 copies (ultrarapid metabolizers) clear it quickly and in studies had more vomiting; poor metabolizers keep much higher levels. CPIC has published a guideline on CYP2D6 for the 5-HT3 antagonists, covering tropisetron, and all the studies on this page concern CYP2D6.

Published guidelines

Clinical guidelines that name a gene for Tropisetron. They sort people into groups by the gene copies they carry.

CPIC guideline: CYP2D6. CYP2D6 activity score 0 to 6 or more. Strong. 2017.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP2D6 CYP2D6*1, CYP2D6*2xN, CYP2D6*3, CYP2D6*4, CYP2D6*5, CYP2D6*6, CYP2D6*10: Level 1A · replicated. Metabolism/PK. 2.

CYP2D6 CYP2D6*1, CYP2D6*1xN, CYP2D6*2xN: Level 1A · replicated. Efficacy: Vomiting. 1.

Metabolism (3 findings)

Studies of how quickly the body processes this medication.

CYP2D6 CYP2D6*1, CYP2D6*10: CYP2D6 *1/*10 is associated with decreased metabolism of tropisetron in healthy individuals as compared to CYP2D6 *1/*1 (assigned as normal metabolizer phenotype) .

CYP2D6 CYP2D6*1, CYP2D6*2xN: CYP2D6 *1/*2xN (assigned as ultrarapid metabolizer phenotype) is associated with increased metabolism of tropisetron in healthy individuals as compared to CYP2D6 *1/*1 (assigned as normal metabolizer phenotype) .

CYP2D6 CYP2D6*1, CYP2D6*5, CYP2D6*10: CYP2D6 *10/*10 + *5/*10 (assigned as poor metabolizer phenotype) is associated with decreased metabolism of tropisetron in healthy individuals as compared to CYP2D6 *1/*1 (assigned as normal metabolizer phenotype) .

Response (2 findings)

Studies of how well it worked in people carrying a variant.

CYP2D6 CYP2D6*1, CYP2D6*1xN: CYP2D6 *1/*1xN is associated with decreased response to ondansetron or tropisetron in people with Neoplasms.

CYP2D6 CYP2D6*3: CYP2D6 *3/*3 is associated with decreased metabolism of tropisetron in people with Neoplasms.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
