Tenofovir alafenamide (TAF) is a newer prodrug of tenofovir that is converted to the active form mainly inside cells, so far less tenofovir circulates in the blood than with the older TDF, with fewer kidney and bone effects. It is used for chronic hepatitis B (Vemlidy) and in many HIV combination tablets, such as Biktarvy and Descovy.
TAF is a substrate of the drug pump ABCB1 in the gut and is activated by cathepsin A and CES1 inside cells; medicines that block or speed up ABCB1 change its levels more than genes do. The gene studies listed looked at ABCB1 and CYP3A4. There is no CPIC or DPWG guideline.
