About Mayzent

What it is

Siponimod is an immunomodulator for multiple sclerosis. It binds sphingosine-1-phosphate (S1P) receptors on white blood cells and keeps many of them inside the lymph nodes, so fewer reach the brain and spinal cord. It is used for relapsing forms of multiple sclerosis, including active secondary progressive MS. It is sold as Mayzent.

How genes come into it

Siponimod is broken down mainly by the liver enzyme CYP2C9, and it is one of the few medicines whose US label asks for a person's CYP2C9 genotype before treatment starts. People with two copies of the reduced-function *3 version have several times the usual exposure, and those with one copy clearly more than average. DPWG has published a guideline on CYP2C9 and siponimod, and the US FDA lists the pair.

Published guidelines

Clinical guidelines that name a gene for Mayzent. They sort people into groups by the gene copies they carry.

DPWG guideline: CYP2C9. Normal Metabolizer · Intermediate Metabolizer · Poor Metabolizer · activity score 0.0 · activity score 0.5 · activity score 1.0 · activity score 1.5 · activity score 2.0 · n/a · *3/*3 · *2/*3 · *1/*3 · *2/*2 · *1/*2 · *3/*9 · *2/*9.

US FDA: CYP2C9. *2/*3 · *1/*2 · *3/*3 · *2/*2 · *1/*1 · *1/*3.

US FDA: CYP2C9. CYP2C9 activity score 0 to 1.5. Data that Support Therapeutic Management Recommendations.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP2C9 CYP2C9*1, CYP2C9*2, CYP2C9*3: Level 1A · replicated. Metabolism/PK. 2.

Metabolism (2 findings)

Studies of how quickly the body processes this medication.

CYP2C9 CYP2C9*1, CYP2C9*2, CYP2C9*3: CYP2C9 *2/*3 + *3/*3 are associated with increased concentrations of siponimod in healthy individuals as compared to CYP2C9 *1/*1.

CYP2C9 CYP2C9*1, CYP2C9*2, CYP2C9*3: CYP2C9 *2/*3 + *3/*3 + *1/*3 are associated with increased concentrations of siponimod as compared to CYP2C9 *1/*1 + *1/*2 + *2/*2.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
