Lenalidomide (Revlimid)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Lenalidomide

What it is

Lenalidomide is an immunomodulatory medicine, a more potent relative of thalidomide. It binds the protein cereblon and makes the cell break down proteins myeloma cells depend on, and it also activates immune cells and blocks blood vessel growth. It is used for multiple myeloma, some myelodysplastic syndromes (especially with a deletion in chromosome 5q) and some lymphomas. It is sold as Revlimid.

How genes come into it

Lenalidomide is cleared mostly unchanged by the kidneys, so liver-enzyme genes play little part. Studies have looked at blood vessel growth genes (VEGFA, KDR, FGF2), CTNNB1 and ABCB1 for response and side effects. Whether it works well in myelodysplastic syndrome depends on the 5q deletion in the abnormal blood cells, which is acquired, not inherited. There is no CPIC or DPWG guideline.

Rated evidence (4 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

ABCB1 rs2229109: Level 3. Efficacy: Multiple Myeloma, Progression-free survival. 1.

CTNNB1 rs4135385: Level 3. Efficacy, Toxicity: Multiple Myeloma. 1.

CTNNB1 rs4533622: Level 3. Efficacy, Toxicity: Multiple Myeloma. 1.

FGF2 rs308395: Level 3. Efficacy: Multiple Myeloma. 1.

Response (5 findings)

Studies of how well it worked in people carrying a variant.

FGF2 rs308395: Genotypes CG + GG is associated with decreased response to lenalidomide or thalidomide in people with Multiple Myeloma as compared to genotype CC.

KDR rs1870377: Genotypes AT + TT is associated with increased response to carfilzomib, dexamethasone and lenalidomide in people with Multiple Myeloma as compared to genotype AA.

KDR rs2305948: Genotypes CT + TT is associated with increased response to carfilzomib, dexamethasone and lenalidomide in people with Multiple Myeloma as compared to genotype CC.

VEGFA rs833061: Genotypes CT + TT is associated with increased response to carfilzomib, dexamethasone and lenalidomide in people with Multiple Myeloma as compared to genotype CC.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
