Midazolam is a short-acting benzodiazepine that strengthens the calming effect of GABA in the brain. It is used for sedation before and during procedures and surgery, in intensive care, and as a mouth or nasal spray to stop prolonged seizures. It also blocks memory formation for a short time. It is sold as Versed, Hypnovel and Buccolam, among other names.
Midazolam is broken down almost entirely by CYP3A4 and CYP3A5, so completely that it is the standard test substance for measuring CYP3A activity in research. Studies have looked at CYP3A4, CYP3A5 and the vitamin D receptor gene VDR, which regulates CYP3A4, for its clearance; the effects found are modest. There is no CPIC or DPWG guideline.
