Mepolizumab (Nucala)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Mepolizumab

What it is

Mepolizumab is a biologic medicine, a monoclonal antibody that neutralises interleukin-5, the main signal that recruits and sustains eosinophils, a type of white blood cell. Injected monthly, it is used for severe eosinophilic asthma, chronic rhinosinusitis with nasal polyps, eosinophilic granulomatosis with polyangiitis and hypereosinophilic syndrome. It is sold as Nucala.

How genes come into it

As an antibody, mepolizumab is not broken down by drug-processing enzymes. The gene studies listed looked at ADRB2 and HLA-DQA1 for response in asthma. The findings come from a few studies, and there is no CPIC or DPWG guideline. Whether it is used depends mostly on the blood eosinophil count, not on genes.

Response (2 findings)

Studies of how well it worked in people carrying a variant.

ADRB2 rs1042713: Allele G is associated with increased clinical benefit to mepolizumab in people with Asthma as compared to allele A.

HLA-DQA1 HLA-DQA1*05: HLA-DQA1 *05 is not associated with decreased clinical benefit to adalimumab, infliximab, mepolizumab or ustekinumab in people with Ulcerative Colitis or Crohn Disease.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
