Esomeprazole (Nexium)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Esomeprazole

What it is

Esomeprazole is a proton pump inhibitor (PPI), the S-form of omeprazole, which the body clears more slowly than the mixture. It switches off the pumps in the stomach lining that release acid, and is used for heartburn and acid reflux, ulcers, Helicobacter pylori treatment and to protect the stomach during NSAID use. It is sold as Nexium, among other names.

How genes come into it

Esomeprazole is broken down by CYP2C19 and CYP3A4. It depends on CYP2C19 somewhat less than omeprazole, but poor metabolizers still have higher levels, which the US FDA lists as an effect on drug levels, and studies have compared healing and Helicobacter cure by CYP2C19 type. CPIC's PPI guideline did not include esomeprazole.

Metabolism (6 findings)

Studies of how quickly the body processes this medication.

CYP2C19 CYP2C19 poor metabolizer: CYP2C19 poor metabolizer is associated with increased exposure to esomeprazole in healthy individuals as compared to CYP2C19 intermediate metabolizer.

CYP2C19 CYP2C19 rapid metabolizer: CYP2C19 rapid metabolizer is associated with decreased exposure to esomeprazole in healthy individuals as compared to CYP2C19 intermediate metabolizer.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3: CYP2C19 *1/*1 is associated with increased metabolism of esomeprazole in healthy individuals as compared to CYP2C19 *2/*2 + *2/*3 + *3/*3.

CYP2C19 CYP2C19*2, CYP2C19*3, CYP2C19*17: CYP2C19 *17/*17 is associated with increased clearance of esomeprazole and pantoprazole in healthy individuals as compared to CYP2C19 *2/*3.

Response (25 findings)

Studies of how well it worked in people carrying a variant.

ABCB1 rs1045642: Genotype AA is associated with decreased clinical benefit to Antibiotics and esomeprazole in children with Helicobacter Infections as compared to genotypes AG + GG.

CYP2C19 CYP2C19 normal metabolizer genotype: CYP2C19 normal metabolizer is associated with decreased response to amoxicillin, clarithromycin and esomeprazole in people with Gastritis as compared to CYP2C19 intermediate metabolizer and poor metabolizer.

CYP2C19 CYP2C19*1, CYP2C19*17: CYP2C19 *17 is associated with decreased response to esomeprazole in children with eosinophilic esophagitis as compared to CYP2C19 *1/*1.

CYP2C19 CYP2C19*1, CYP2C19*2: CYP2C19 *1/*1 is associated with decreased response to esomeprazole in healthy individuals as compared to CYP2C19 *1/*2.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
