Clobazam (Onfi)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Clobazam

What it is

Clobazam is a benzodiazepine of the 1,5-benzodiazepine type that strengthens the calming effect of GABA with somewhat less sedation than classic benzodiazepines. It is used mainly as an add-on for seizures in Lennox-Gastaut syndrome and other epilepsies, and in some countries for anxiety. It is sold as Onfi, Sympazan and Frisium.

How genes come into it

Clobazam is turned by CYP3A4 into N-desmethylclobazam, which is also active and reaches higher levels than clobazam itself; CYP2C19 then breaks that down. In CYP2C19 poor metabolizers, N-desmethylclobazam builds up to several times the usual level, which the US label addresses and the US FDA lists. There is no CPIC or DPWG guideline; the studies on this page mostly concern CYP2C19.
