Vinorelbine (Navelbine)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Vinorelbine

What it is

Vinorelbine is a vinca alkaloid chemotherapy medicine, a semi-synthetic relative of vincristine from the periwinkle plant. It blocks the assembly of microtubules, so dividing cells cannot separate their chromosomes. It is used for non-small cell lung cancer and advanced breast cancer, by drip or as capsules. It is sold as Navelbine, among other names.

How genes come into it

Vinorelbine is broken down mainly by CYP3A4. The only gene studies on this page come from one study of chemotherapy combinations in lung cancer, which linked variants in CASP7, a gene of programmed cell death, to response. The evidence is weak, and there is no CPIC or DPWG guideline.

Rated evidence (4 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CASP7 rs2227310: Level 3. Efficacy: Non-Small Cell Lung Carcinoma. 1.

CASP7 rs4353229: Level 3. Efficacy: Non-Small Cell Lung Carcinoma. 1.

CASP7 rs7921977: Level 3. Efficacy: Non-Small Cell Lung Carcinoma. 1.

CASP7 rs12415607: Level 3. Efficacy: Non-Small Cell Lung Carcinoma. 1.

Response (5 findings)

Studies of how well it worked in people carrying a variant.

CASP7 rs12415607: Genotypes AA + AC are associated with increased response to docetaxel, gemcitabine, paclitaxel, Platinum compounds or vinorelbine in people with Carcinoma, Non-Small-Cell Lung as compared to genotype CC.

CASP7 rs2227310: Genotypes CG + GG are associated with increased response to docetaxel, gemcitabine, paclitaxel, Platinum compounds or vinorelbine in people with Carcinoma, Non-Small-Cell Lung as compared to genotype CC.

CASP7 rs4353229: Genotypes CC + CT are associated with increased response to docetaxel, gemcitabine, paclitaxel, Platinum compounds or vinorelbine in people with Carcinoma, Non-Small-Cell Lung as compared to genotype TT.

CASP7 rs7921977: Genotype CT is associated with decreased response to docetaxel, gemcitabine, paclitaxel, Platinum compounds or vinorelbine in people with Carcinoma, Non-Small-Cell Lung as compared to genotype TT.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
