Ticagrelor is an antiplatelet medicine: it blocks the P2Y12 receptor on platelets, so they are less likely to clump and form a clot. Unlike clopidogrel, it works directly, without needing to be activated by the liver. It is used with aspirin after a heart attack or unstable angina, and after stenting. It is sold as Brilinta and Brilique.
Because ticagrelor does not need CYP2C19 to work, it is one of the alternatives clinical guidelines name for people whose CYP2C19 genes make clopidogrel less effective. Ticagrelor itself is broken down by CYP3A4 into an active form and carried by SLCO1B1; studies have linked variants in these to its levels, and the platelet gene PEAR1 to its effect. These findings are weak, and there is no CPIC or DPWG guideline for ticagrelor itself.
