Prasugrel (Effient)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Prasugrel

What it is

Prasugrel is an antiplatelet medicine of the thienopyridine family, like clopidogrel: it permanently blocks the P2Y12 receptor on platelets so they are less likely to clump. It is used with aspirin in people with acute coronary syndrome who are treated with a stent. It is sold as Effient and Efient.

How genes come into it

Prasugrel is also a prodrug, but it is activated in a single step mainly by CYP3A4 and CYP2B6, so CYP2C19 variants hardly affect it. That is why clinical guidelines name it, with ticagrelor, as an alternative for people whose CYP2C19 genes make clopidogrel less effective. Studies on prasugrel itself confirmed that CYP2C19 has little effect and looked at the platelet gene PEAR1, with weak evidence. There is no CPIC or DPWG guideline for prasugrel itself.

Metabolism (5 findings)

Studies of how quickly the body processes this medication.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*17: CYP2C19 *1/*2 + *2/*2 are not associated with decreased metabolism of prasugrel in people with Coronary Artery Disease as compared to CYP2C19 *1/*1 + *1/*17 + *17/*17.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3: CYP2C19 *2/*2 + *2/*3 are not associated with metabolism of prasugrel in healthy individuals as compared to CYP2C19 *1/*1.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3, CYP2C19*8, CYP2C19*17: CYP2C19 *1/*2 + *1/*8 + *2/*2 + *2/*3 is not associated with metabolism of prasugrel in people with Coronary Artery Disease as compared to CYP2C19 *1/*1 + *1/*17 + *17/*17.

CYP3A4 CYP3A4*1, CYP3A4*22: CYP3A4 *1/*22 is not associated with exposure to prasugrel in healthy individuals as compared to CYP3A4 *1/*1.

Response (11 findings)

Studies of how well it worked in people carrying a variant.

CYP2C19 rs4244285: Allele A is associated with response to clopidogrel, prasugrel and ticagrelor in people with Acute coronary syndrome.

PEAR1 rs12041331: Genotypes AA + AG are associated with increased response to aspirin, clopidogrel or prasugrel as compared to genotype GG.

PEAR1 rs2768759: Genotypes AC + CC are associated with decreased response to aspirin or prasugrel as compared to genotype AA.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3: CYP2C19 *1/*1 + *1/*2 + *1/*3 is associated with response to clopidogrel and prasugrel in people with Myocardial Infarction.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
