About Thiotepa

What it is

Thiotepa is an alkylating chemotherapy medicine that cross-links DNA. At high doses it is used to prepare the bone marrow before stem cell transplants, especially in children with blood disorders and in some brain tumours; it is also placed directly in the bladder for some bladder cancers. It is sold as Tepadina, among other names.

How genes come into it

Thiotepa is converted by CYP2B6 and CYP3A4 into TEPA, also active, and both are inactivated by joining to glutathione with the help of glutathione transferases. The gene studies listed looked at GSTP1 for its clearance and at ALDH1A1 for side effects. The findings come from a few studies, and there is no CPIC or DPWG guideline.
