About Alkylating Agents

What it is

Alkylating agents are one of the oldest classes of chemotherapy medicines. They attach alkyl groups to DNA and cross-link its strands, so cancer cells cannot copy it and die. The class includes cyclophosphamide, ifosfamide, melphalan, chlorambucil, busulfan, bendamustine, temozolomide and the nitrosoureas, used for leukaemias, lymphomas, myeloma, breast cancer, brain tumours and before stem cell transplants. This page collects studies on the class as a group.

How genes come into it

The few studies on alkylating agents as a group looked at EGFR and ESR1 for response and side effects. The more specific gene findings belong to individual medicines and tumours: GSTA1 for busulfan clearance, CYP2B6 for activating cyclophosphamide, and MGMT switched off in the tumour for temozolomide. None of these has a CPIC or DPWG guideline.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

EGFR rs712829: Level 3. Efficacy: Neoplasms. 1.

ESR1 rs2207396: Level 3. Toxicity: Neoplasms. 1.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
