Rasagiline (Azilect)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Rasagiline

What it is

Rasagiline is an MAO-B inhibitor: it permanently blocks monoamine oxidase B, an enzyme that breaks down dopamine in the brain, so dopamine lasts longer. It is used for Parkinson's disease, alone in early stages or with levodopa to smooth out 'off' periods. It is sold as Azilect.

How genes come into it

Rasagiline is broken down mainly by the liver enzyme CYP1A2. Studies have looked at CYP1A2, the transporters SLC22A1, ABCB1 and ABCC2, and COMT for its blood levels, and at the dopamine receptor gene DRD2 for response. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

DRD2 rs1076560: Level 3. Efficacy: Parkinson Disease. 1.

DRD2 rs2283265: Level 3. Efficacy: Parkinson Disease. 1.

Metabolism (6 findings)

Studies of how quickly the body processes this medication.

ABCB1 rs1045642: Genotypes AA + AG is associated with decreased clearance of rasagiline in healthy individuals as compared to genotype GG.

ABCC2 rs2273697: Genotype AA is associated with decreased clearance of rasagiline in healthy individuals as compared to genotypes AG + GG.

COMT rs4680: Genotype AA is associated with decreased concentrations of rasagiline in healthy individuals as compared to genotypes AG + GG.

SLC22A1 rs12208357: Genotype CT is associated with decreased clearance of rasagiline in healthy individuals as compared to genotype CC.

Response (2 findings)

Studies of how well it worked in people carrying a variant.

DRD2 rs1076560: Genotype CC is associated with increased response to rasagiline in people with Parkinson Disease as compared to genotypes AA + AC.

DRD2 rs2283265: Genotype CC is associated with increased response to rasagiline in people with Parkinson Disease as compared to genotypes AA + AC.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
