Ranibizumab (Lucentis)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Ranibizumab

What it is

Ranibizumab is a fragment of a monoclonal antibody that neutralises VEGF, the signal that drives abnormal, leaky blood vessel growth in the eye. Injected into the eye, it is used for wet age-related macular degeneration, diabetic macular oedema, swelling after retinal vein occlusion and retinopathy of prematurity. It is sold as Lucentis and as biosimilars.

How genes come into it

Studies on ranibizumab have looked for genes that predict how much vision improves after eye injections. The most studied are VEGFA and its receptor KDR, and genes linked to macular degeneration itself, CFH, ARMS2 and HTRA1. Results differ between studies and none is used in treatment, and there is no CPIC or DPWG guideline.

Rated evidence (7 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

NRP1 rs2070296: Level 3. Efficacy: Macular Degeneration. 1.

VEGFA rs3025000: Level 3. Efficacy: Macular Degeneration. 1.

VEGFA rs2010963: Level 3. Efficacy: Choroidal Neovascularization. 1.

CFH rs1061170: Level 4. Efficacy: Macular Degeneration. 10.

Response (78 findings)

Studies of how well it worked in people carrying a variant.

ANO2 rs2110166: Genotype TT is associated with increased clinical benefit to aflibercept or ranibizumab in people with Macular Degeneration as compared to genotypes CC + CT.

CFH rs1061170: Genotype TT is associated with increased response to ranibizumab in people with Macular Degeneration as compared to genotype CC.

CFH rs1061170: Genotype CC is associated with decreased response to bevacizumab and ranibizumab in people with Macular Degeneration as compared to genotypes CT + TT.

CXCL8 rs4073: Genotype AA is associated with decreased response to ranibizumab in people with Macular Degeneration as compared to genotypes AT + TT.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
