Procainamide (Pronestyl)

About Procainamide

What it is

Procainamide is a class 1A antiarrhythmic: it blocks sodium channels in the heart and slows conduction. It is given by drip in hospital to stop some fast heart rhythms, such as certain ventricular tachycardias and atrial fibrillation with an accessory pathway. Long-term use by mouth is now rare because of side effects. It was sold as Pronestyl.

How genes come into it

Procainamide is converted by NAT2 into N-acetylprocainamide (NAPA), which also affects the heart rhythm. Slow acetylators, with two low-activity NAT2 copies, keep more procainamide itself and develop a drug-induced lupus earlier and more often on long-term treatment; the US FDA lists the pair as affecting safety. There is no CPIC or DPWG guideline.
