Prednisone

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Prednisone

What it is

Prednisone is a corticosteroid prodrug that the liver turns into prednisolone, which calms inflammation and the immune system through the glucocorticoid receptor. It is one of the most used steroid tablets, for asthma and COPD flare-ups, rheumatoid arthritis, polymyalgia, inflammatory bowel disease, autoimmune conditions, allergic reactions and, in combinations such as CHOP, blood cancers. It is sold as Deltasone, among other names.

How genes come into it

Prednisone is activated by 11-beta-hydroxysteroid dehydrogenase and then broken down by CYP3A4. Studies have looked at NR3C1, the gene for the glucocorticoid receptor, for response, and at ABCB1 and other genes for side effects, partly in children treated for leukaemia. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (13 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

GSTA1 rs3957357: Level 3. Efficacy: Lymphoma, Large B-Cell, Diffuse. 2.

ABCB1 rs1045642: Level 3. Other: Organ Transplantation, Transplantation. 1.

CTLA4 rs4553808: Level 3. Toxicity. 1.

GATA3 rs3824662: Level 3. Efficacy. 1.

Response (5 findings)

Studies of how well it worked in people carrying a variant.

CXCL12 rs1801157: Genotypes CT + TT are associated with decreased response to alemtuzumab, chlorambucil, cyclophosphamide, fludarabine, prednisone, rituximab or vincristine in people with Leukemia, Lymphocytic, Chronic, B-Cell as compared to genotype CC.

EDNRA rs5333: Genotypes CC + CT is associated with increased resistance to prednisone in children with Nephrotic Syndrome as compared to genotype TT.

NR3C1 rs10052957: Allele A is associated with decreased resistance to prednisone in people with Nephrotic Syndrome as compared to allele G.

NR3C1 rs258751: Allele A is associated with decreased resistance to prednisone in people with Nephrotic Syndrome as compared to allele G.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
