Ledipasvir is a direct-acting antiviral that blocks the hepatitis C protein NS5A. It is given only combined with sofosbuvir in one tablet (Harvoni), which cures most genotype 1 hepatitis C infections in 8 to 12 weeks without interferon.
Ledipasvir is barely broken down and is excreted mostly unchanged in bile. The gene studies listed looked at the IFNL3 (IL28B) region, which strongly predicted cure with older interferon treatment; with ledipasvir and sofosbuvir its effect on cure rates is small. There is no CPIC or DPWG guideline.
