Dabigatran is a direct oral anticoagulant (DOAC) that blocks thrombin, the final enzyme of the clotting cascade. It does not need regular blood tests, and a specific antidote (idarucizumab) can reverse it. It is used to lower the chance of stroke in atrial fibrillation and to prevent and treat clots in the legs and lungs. It is sold as Pradaxa.
Dabigatran is taken as a prodrug that the esterase CES1 converts into the active form; it is not broken down by CYP enzymes, and the kidneys clear it. In the large RE-LY trial, a CES1 variant (rs2244613) was linked to lower levels and less bleeding, and ABCB1 variants to higher levels. The effects are modest, and there is no CPIC or DPWG guideline.
