About Cannabinoids

What it is

Cannabinoids are substances that act on the body's cannabinoid receptors (CB1 and CB2). They include THC and cannabidiol (CBD) from the cannabis plant and medicines made from them: dronabinol and nabilone for nausea from chemotherapy and appetite loss, nabiximols (Sativex) for muscle stiffness in multiple sclerosis, and cannabidiol (Epidiolex) for some severe childhood epilepsies. This page collects studies on cannabinoids as a group.

How genes come into it

THC is broken down mainly by CYP2C9, and people with the reduced-function CYP2C9*3 version have shown higher THC levels; cannabidiol is broken down by CYP2C19 and CYP3A4. The gene studies on this page, mostly on cannabis dependence, looked at OPRM1, FAAH (which breaks down the body's own cannabinoids) and others. The findings are weak, and there is no CPIC or DPWG guideline.

Rated evidence (4 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

KAT2B rs9829896: Level 3. Toxicity: Substance-Related Disorders. 1.

OPRM1 rs1799971: Level 3. Toxicity: Marijuana Abuse. 1.

FAAH rs324420: Level 3. Toxicity: Substance-Related Disorders. 1.

OPRM1 rs1799971: Level 4. Toxicity: Substance-Related Disorders. 5.

Response (3 findings)

Studies of how well it worked in people carrying a variant.

ABCB1 rs1045642: Genotypes AG + GG is associated with increased clinical benefit to cannabinoids in people with Pain as compared to genotype AA.

TRPV1 rs8065080: Genotype CC is associated with increased clinical benefit to cannabinoids in people with Pain as compared to genotypes CT + TT.

UGT2B7 rs7438135: Genotype AA is associated with increased clinical benefit to cannabinoids in people with Pain as compared to genotypes AG + GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
