Cannabidiol (CBD)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Cannabidiol

What it is

Cannabidiol (CBD) is a non-intoxicating compound from the cannabis plant. As the prescription medicine Epidiolex (Epidyolex) it is used for seizures in Dravet syndrome, Lennox-Gastaut syndrome and tuberous sclerosis complex; it is also widely sold as a supplement, where quality and content vary. How it reduces seizures is not fully understood.

How genes come into it

Cannabidiol is broken down mainly by CYP2C19 and CYP3A4, and it blocks CYP2C19 itself, notably raising the active breakdown product of clobazam when the two are combined. The gene studies listed, from a study in children with epilepsy, looked at ABCC5, AOC1 and SLC15A1 for response. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Response (4 findings)

Studies of how well it worked in people carrying a variant.

rs6729738: Genotype CC is associated with increased response to cannabidiol in people with Epilepsy as compared to genotypes AA + AC.

ABCC5 rs3749442: Genotypes AA + AG are associated with decreased response to cannabidiol in people with Epilepsy as compared to genotype GG.

AOC1 rs12539: Genotypes CT + TT are associated with increased response to cannabidiol in people with Epilepsy as compared to genotype CC.

SLC15A1 rs1339067: Genotype TT is associated with decreased response to cannabidiol in people with Epilepsy as compared to genotypes AA + AT.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
