Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Bucindolol

What it is

Bucindolol is an experimental beta blocker with mild vasodilating properties. It was tested in heart failure in the BEST trial, which did not show an overall survival benefit, and is being developed for atrial fibrillation in people with a particular beta-1 receptor genotype. It is not approved for routine use.

How genes come into it

Bucindolol is a prominent example of a medicine developed around a gene. Analyses of the BEST trial found that people with two copies of the Arg389 version of ADRB1, the beta-1 receptor gene, benefited markedly while others did not, and the ADRA2C deletion variant also modified the effect. A later trial (GENETIC-AF) compared it with metoprolol in this genotype group. There is no CPIC or DPWG guideline.

Rated evidence (2 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

ADRB1 rs1801253: Level 3. Efficacy: Heart Failure. 2.

ADRA2C rs61767072: Level 3. Efficacy: Heart Failure. 1.

Response (2 findings)

Studies of how well it worked in people carrying a variant.

ADRA2C rs61767072: Genotype GGGGCGGGGCCG/GGGGCGGGGCCG is associated with increased response to bucindolol in people with Heart Failure as compared to genotypes GGGGCGGGGCCG/del + del/del.

ADRB1 rs1801253: Genotype CC is associated with increased response to bucindolol in people with Heart Failure as compared to genotypes CG + GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
