Bisoprolol is a beta blocker that acts very selectively on the heart (beta-1): it blocks adrenaline there, slowing the heart rate and lowering blood pressure. It is used for high blood pressure, angina, heart failure and to control the heart rate in atrial fibrillation. It is sold as Concor, Zebeta and Cardicor, among other names.
Bisoprolol is cleared half by the kidneys unchanged and half by the liver, mainly by CYP3A4 with a small part by CYP2D6, so CYP2D6 variants matter much less than for metoprolol. Studies have looked at CYP2D6, CYP3A5 and the beta-1 receptor gene ADRB1 for heart rate and blood pressure response. The findings come from a few studies, and there is no CPIC or DPWG guideline.
