Bevacizumab (Avastin)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Bevacizumab

What it is

Bevacizumab is a biologic medicine, a monoclonal antibody that neutralises VEGF, the signal tumours use to grow new blood vessels. Given by drip, it is used with chemotherapy for bowel, lung, ovarian, cervical, kidney and brain cancers; as an injection into the eye, it is used off label for wet age-related macular degeneration. It is sold as Avastin and as biosimilars.

How genes come into it

As an antibody, bevacizumab is not broken down by drug-processing enzymes. Studies in cancer have looked at variants in VEGFA and its pathway (EPAS1, ANGPT2) for how well it worked. Studies in macular degeneration have looked at CFH and ARMS2, genes linked to that eye condition, for response to eye injections. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (24 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

MTHFR rs1801131: Level 3. Efficacy: Neoplasms. 4.

VEGFA rs3025039: Level 3. Efficacy: Breast Neoplasms, Colorectal Neoplasms. 3.

ARMS2 rs10490924: Level 3. Efficacy: Macular Degeneration. 2.

VEGFA rs699947: Level 3. Efficacy: Colorectal Neoplasms. 2.

Response (97 findings)

Studies of how well it worked in people carrying a variant.

ARMS2 rs10490924: Genotype TT is associated with decreased response to bevacizumab in people with Macular Degeneration as compared to genotype GG.

ARMS2 rs10490924: Genotypes GT + TT are associated with increased response to bevacizumab in people with Macular Degeneration as compared to genotype GG.

CFH rs1061170: Genotype CC is associated with decreased response to bevacizumab and ranibizumab in people with Macular Degeneration as compared to genotypes CT + TT.

CFH rs1061170: Genotype CC is associated with decreased response to bevacizumab in people with Macular Degeneration as compared to genotypes CT + TT.

Dosing (2 findings)

Studies of how much of the drug people needed or tolerated.

CFH rs1061170: Genotype CT is associated with increased dose of bevacizumab in people with Macular Degeneration as compared to genotype TT.

ARMS2 rs10490924: Genotypes GT + TT are not associated with dose of bevacizumab in people with Macular Degeneration as compared to genotype GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
