Amlodipine (Norvasc)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Amlodipine

What it is

Amlodipine is a calcium channel blocker of the dihydropyridine group: it relaxes the muscle in artery walls, so they widen and blood pressure falls. It lasts long enough to be taken once a day and is one of the most prescribed medicines for high blood pressure and angina. It is sold as Norvasc and Istin, among other names.

How genes come into it

Amlodipine is broken down mainly by CYP3A4, with CYP3A5 contributing. Studies have looked at variants in both for blood levels and how much blood pressure fell, and at the calcium channel gene CACNA1C and renin-angiotensin genes such as AGT and ACE. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (9 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

Metabolism (7 findings)

Studies of how quickly the body processes this medication.

ABCB1 rs1045642: Genotype GG is associated with decreased clearance of amlodipine in healthy individuals as compared to genotypes AA + AG.

ABCB1 rs2032582: Genotype CC is associated with decreased clearance of amlodipine in healthy individuals as compared to genotypes AA + AC.

CYP3A4 CYP3A4*1, CYP3A4*22: CYP3A4 *1/*22 + *22/*22 is associated with increased dose-adjusted trough concentrations of amlodipine in people with Hypertension as compared to CYP3A4 *1/*1.

CYP3A5 rs776746: Genotype CC is associated with increased metabolism of amlodipine in healthy individuals as compared to genotypes CT + TT.

Response (18 findings)

Studies of how well it worked in people carrying a variant.

CACNA1C rs2238032: Genotype TT is associated with increased response to amlodipine or felodipine in people with Hypertension as compared to genotypes GG + GT.

CACNA1C rs2239050: Genotype GG is associated with increased response to amlodipine and felodipine in people with Hypertension as compared to genotype CG.

CACNA1C rs2239050: Genotype GG is associated with increased clinical benefit to amlodipine in people with Hypertension as compared to genotypes CC + CG.

CACNA1C rs2239128: Allele C is associated with increased response to amlodipine and felodipine in people with Hypertension as compared to allele T.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
