Zuclopenthixol (Clopixol)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Zuclopenthixol

What it is

Zuclopenthixol is a first-generation antipsychotic of the thioxanthene group that blocks dopamine D1 and D2 receptors. It is used for schizophrenia and other psychoses, as tablets, as a short-acting injection for acute episodes, and as a long-acting injection. It is sold as Clopixol and Cisordinol, among other names; it is not marketed in the US.

How genes come into it

Zuclopenthixol is broken down mainly by the liver enzyme CYP2D6. People with little or no CYP2D6 activity have clearly higher levels, and people with extra copies lower ones. DPWG has published a guideline on CYP2D6 and zuclopenthixol, and all the studies on this page concern CYP2D6.

Published guidelines

Clinical guidelines that name a gene for Zuclopenthixol. They sort people into groups by the gene copies they carry.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP2D6 CYP2D6*1, CYP2D6*3, CYP2D6*4: Level 1A · replicated. Metabolism/PK: Schizophrenia. 2.

Metabolism (8 findings)

Studies of how quickly the body processes this medication.

CYP2D6 CYP2D6 intermediate metabolizer: CYP2D6 intermediate metabolizer and poor metabolizer are associated with increased concentrations of zuclopenthixol in people with Mental Disorders as compared to CYP2D6 normal metabolizer.

CYP2D6 CYP2D6 normal metabolizer: CYP2D6 normal metabolizer and ultrarapid metabolizer is associated with decreased dose-adjusted trough concentrations of zuclopenthixol as compared to CYP2D6 intermediate metabolizer and poor metabolizer.

CYP2D6 CYP2D6*1, CYP2D6*3: CYP2D6 *3 is associated with decreased metabolism of zuclopenthixol in people with Schizophrenia as compared to CYP2D6 *1.

CYP2D6 CYP2D6*1, CYP2D6*3, CYP2D6*4: CYP2D6 *1/*4 + *3/*4 are associated with decreased metabolism of zuclopenthixol in people with Schizophrenia as compared to CYP2D6 *1/*1.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
