Vortioxetine (Trintellix)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Vortioxetine

What it is

Vortioxetine is an antidepressant with a mixed action: it blocks the reuptake of serotonin and also acts directly on several serotonin receptors. It is used for major depression in adults. It is sold as Trintellix and Brintellix, among other names.

How genes come into it

Vortioxetine is broken down mainly by the liver enzyme CYP2D6. People with no working CYP2D6 copies (poor metabolizers) have roughly twice the blood level of people with normal activity, and the US drug label addresses them. CPIC has published a guideline on CYP2D6 and vortioxetine, and the US FDA lists the pair. Only a few studies on this medicine have looked at genes.

Published guidelines

Clinical guidelines that name a gene for Vortioxetine. They sort people into groups by the gene copies they carry.

CPIC guideline: CYP2D6. CYP2D6 activity score 0 to 6 or more. Strong. 2023.

US FDA: CYP2D6. CYP2D6 activity score 0.

US FDA: CYP2D6. CYP2D6 activity score 0. Data that Support Therapeutic Management Recommendations.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP2D6 CYP2D6*1, CYP2D6*4, CYP2D6*10: Level 1A · replicated. Metabolism/PK. 2.

Metabolism (2 findings)

Studies of how quickly the body processes this medication.

CYP2D6 CYP2D6*1, CYP2D6*4, CYP2D6*10: CYP2D6 *4/*4 + *4/*10 is associated with increased concentrations of vortioxetine as compared to CYP2D6 *1/*1.

CYP2D6 CYP2D6 poor metabolizer: CYP2D6 poor metabolizer is associated with decreased clearance of vortioxetine in healthy individuals as compared to CYP2D6 normal metabolizer.

Side effects (1 finding)

Studies of side effects in people carrying a variant.

CYP2D6 CYP2D6*1, CYP2D6*4: CYP2D6 *4/*4 (assigned as poor metabolizer phenotype) is associated with increased discontinuation of vortioxetine as compared to CYP2D6 *1/*1 (assigned as normal metabolizer phenotype) .

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
