Topiramate (Topamax)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Topiramate

What it is

Topiramate is an anticonvulsant with several actions: it blocks sodium channels and AMPA/kainate glutamate receptors, strengthens GABA and weakly inhibits carbonic anhydrase. It is used for epilepsy, to prevent migraine and, combined with phentermine (Qsymia), for weight management. Tingling, word-finding difficulty and kidney stones are known side effects. It is sold as Topamax, among other names.

How genes come into it

Topiramate is cleared mostly unchanged by the kidneys. Studies have looked at GRIK1, a kainate receptor gene, for how well it reduced heavy drinking (an off-label use), at GABA receptor genes and SCN2A for seizures, and at CA12 for side effects. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Metabolism (2 findings)

Studies of how quickly the body processes this medication.

GRIK1 rs2832407: Genotypes AA + AC is associated with increased concentrations of topiramate in people with Alcohol-Related Disorders as compared to genotype CC.

Response (14 findings)

Studies of how well it worked in people carrying a variant.

ABCB1 rs1045642: Genotype AA is associated with increased response to topiramate in people with Migraine without Aura or Migraine with Aura as compared to genotypes AG + GG.

GRIK1 rs2832407: Genotypes AA + AC is associated with decreased response to topiramate in people with Alcohol-Related Disorders as compared to genotype CC.

rs511310: Genotype GG is not associated with decreased response to carbamazepine, gabapentin, lamotrigine, oxcarbazepine, phenytoin, topiramate or valproic acid in people with Epilepsy as compared to genotype AA.

CYP2C19 CYP2C19*1, CYP2C19*2: CYP2C19 *2 is not associated with response to amitriptyline, propranolol, topiramate or valproic acid in people with Migraine without Aura or Migraine with Aura as compared to CYP2C19 *1.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
