Tildrakizumab (Ilumya)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Tildrakizumab

What it is

Tildrakizumab is a biologic medicine, a monoclonal antibody that blocks the p19 part of interleukin-23, a signal that drives the inflammation in psoriasis. Injected every twelve weeks after two starting doses, it is used for moderate to severe plaque psoriasis. It is sold as Ilumya and Ilumetri.

How genes come into it

As an antibody, tildrakizumab is not broken down by drug-processing enzymes. The few gene studies on it, from one study of psoriasis patients, looked at immune genes such as TNFAIP3, IL17RC, ATG5 and PDE4D for who responded. The findings have not been confirmed, and there is no CPIC or DPWG guideline.

Response (4 findings)

Studies of how well it worked in people carrying a variant.

ATG5 rs9373839: Genotype CT is associated with decreased clinical benefit to tildrakizumab in people with Psoriasis as compared to genotypes CC + TT.

IL17RC rs708567: Genotype CC is associated with increased clinical benefit to tildrakizumab in people with Psoriasis as compared to genotypes CT + TT.

PDE4D rs10556657: Genotype CT/CT is associated with decreased clinical benefit to tildrakizumab in people with Psoriasis as compared to genotypes CT/CTGTCT + CTGTCT/CTGTCT.

TNFAIP3 rs610604: Genotype GG is associated with decreased clinical benefit to tildrakizumab in people with Psoriasis as compared to genotypes GT + TT.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
