Thioridazine (Mellaril)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Thioridazine

What it is

Thioridazine is an older phenothiazine antipsychotic that blocks dopamine D2 and many other receptors. It was widely used for schizophrenia, but because it lengthens the QT interval of the heartbeat more than most antipsychotics, it is now reserved for people who have not responded to others, and has been withdrawn in many countries. It was sold as Mellaril.

How genes come into it

Thioridazine is broken down mainly by CYP2D6. People with little or no CYP2D6 activity reach higher levels, with a greater effect on the heart rhythm, and the US label states that it is contraindicated in people known to have reduced CYP2D6 activity; the US FDA lists the pair. There is no CPIC or DPWG guideline.

Published guidelines

Clinical guidelines that name a gene for Thioridazine. They sort people into groups by the gene copies they carry.

US FDA: CYP2D6. CYP2D6 activity score 0 to 1.

US FDA: CYP2D6. CYP2D6 activity score 0. Data that Support Therapeutic Management Recommendations.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP2D6 rs1080985: Level 3. Other. 1.

Metabolism (4 findings)

Studies of how quickly the body processes this medication.

CYP2D6 CYP2D6*1, CYP2D6*3, CYP2D6*4, CYP2D6*5, CYP2D6*6: CYP2D6 *3/*4 + *4/*4 + *4/*5 + *4/*6 are associated with increased concentrations of thioridazine in people with Mental Disorders as compared to CYP2D6 *1/*1 + *1/*3 + *1/*4 + *1/*5 + *1/*6.

CYP2D6 rs1080985: Genotypes CC + CG is associated with decreased metabolism of thioridazine as compared to genotype GG.

CYP2D6 CYP2D6 poor metabolizer: CYP2D6 poor metabolizer is associated with increased concentrations of thioridazine in people with Depression as compared to CYP2D6 normal metabolizer.

CYP2C19 CYP2C19*1, CYP2C19*2: CYP2C19 *1/*2 + *2/*2 are not associated with concentrations of mesoridazine, sulforidazine and thioridazine in people with Mental Disorders as compared to CYP2C19 *1/*1.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
