Rocuronium (Zemuron)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Rocuronium

What it is

Rocuronium is a non-depolarising muscle relaxant: it blocks acetylcholine receptors at the junction between nerve and muscle, relaxing skeletal muscles during anaesthesia so a breathing tube can be placed and surgery performed. Its effect can be reversed quickly with sugammadex. It is sold as Zemuron and Esmeron, among other names.

How genes come into it

Rocuronium is taken up by the liver through the transporters SLCO1A2 and SLCO1B1 and excreted in bile. Studies, mostly from China, linked SLCO1A2 and SLCO1B1 variants to how long its effect lasted and to its levels. The findings come from a few studies, and there is no CPIC or DPWG guideline.

SLCO1A2 rs11568563: Genotypes GT + TT are not associated with clearance of rocuronium in healthy individuals as compared to genotype GG.

SLCO1A2 rs3764043: Genotypes CT + TT are not associated with clearance of rocuronium in healthy individuals as compared to genotype CC.

SLCO1A2 rs4148978: Genotypes CT + TT are not associated with clearance of rocuronium in healthy individuals as compared to genotype CC.

Response (2 findings)

Studies of how well it worked in people carrying a variant.

ABCB1 rs1128503: Genotype AA is associated with decreased response to rocuronium as compared to genotypes AG + GG.

SLCO1B1 rs2306283: Genotype AA is associated with decreased response to rocuronium as compared to genotypes AG + GG.

Dosing (2 findings)

Studies of how much of the drug people needed or tolerated.

IAPP, SLCO1A2 rs11045995: Genotype CC is associated with decreased dose of rocuronium in women as compared to genotypes CT + TT.

SLCO1A2 rs7967354: Genotype CC is associated with decreased dose of rocuronium in women as compared to genotypes CT + TT.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
