Proguanil

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Proguanil

What it is

Proguanil is an antimalarial, a prodrug that the body turns into cycloguanil, which blocks the parasite's dihydrofolate reductase; proguanil itself also helps atovaquone work. It is mostly used combined with atovaquone (Malarone) to prevent and treat malaria in travellers.

How genes come into it

Proguanil is activated to cycloguanil mainly by CYP2C19, and CYP2C19 poor metabolizers, more common in East Asian and Pacific populations, form little cycloguanil. Since atovaquone-proguanil works largely through proguanil itself and atovaquone, this matters less for the combination. Studies also looked at SLC22A1 for its uptake. There is no CPIC or DPWG guideline.

Metabolism (4 findings)

Studies of how quickly the body processes this medication.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3: CYP2C19 *2/*2 + *2/*3 + *3/*3 is associated with decreased metabolism of proguanil in children with Malaria as compared to CYP2C19 *1/*1.

SLC22A1 rs12208357: Allele T are not associated with concentrations of proguanil in healthy individuals as compared to allele C.

SLC22A1 rs34130495: Allele A are not associated with concentrations of proguanil in healthy individuals as compared to allele G.

SLC22A1 rs2282143: Genotype CT is associated with increased exposure to proguanil in healthy individuals as compared to genotype CC.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
