Perampanel (Fycompa)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Perampanel

What it is

Perampanel is an anticonvulsant that blocks AMPA glutamate receptors, the first medicine of its kind. It is used once daily at bedtime for focal seizures and primary generalised tonic-clonic seizures. Mood and behaviour changes, including irritability and aggression, are known side effects. It is sold as Fycompa.

How genes come into it

Perampanel is broken down mainly by CYP3A4 (with CYP3A5) and lasts a long time in the body. The gene studies on it, mostly from Japan, looked at CYP3A4 and CYP3A5 variants for its blood levels and response. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Metabolism (2 findings)

Studies of how quickly the body processes this medication.

CYP3A5, ZSCAN25 rs776746: Allele C is associated with increased concentrations of perampanel in children with Epilepsy as compared to allele T.

CYP3A5 CYP3A5*1, CYP3A5*3: CYP3A5 *3 is not associated with decreased clearance of perampanel in children with Epilepsy as compared to CYP3A5 *1.

Response (2 findings)

Studies of how well it worked in people carrying a variant.

CYP3A4 rs2242480: Genotypes CT + TT is associated with decreased clinical benefit to perampanel in children with Epilepsy as compared to genotype CC.

CYP3A4 rs4986908: Genotype CG is associated with decreased clinical benefit to perampanel in children with Epilepsy as compared to genotype CC.

Other findings (1 finding)

Studies that do not fall into the groups above.

CYP3A5 CYP3A5*1, CYP3A5*3: CYP3A5 *3/*3 is associated with increased dose-adjusted trough concentrations of perampanel in children with Epilepsy as compared to CYP3A5 *1/*3.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
