About Krystexxa

What it is

Pegloticase is a urate oxidase enzyme attached to polyethylene glycol (PEG) so it lasts longer in the blood. Given by drip every two weeks, it breaks uric acid down into allantoin, which the kidneys clear easily. It is used for severe, long-lasting gout that other uric-acid-lowering medicines have not controlled. It is sold as Krystexxa.

How genes come into it

Like rasburicase, pegloticase releases hydrogen peroxide as it breaks down uric acid, and in people with G6PD deficiency this can cause haemolysis and methaemoglobinaemia. CPIC has published a guideline on G6PD that covers pegloticase, and the US label calls for G6PD screening in people at higher risk, such as those of African, Mediterranean or South Asian ancestry, before treatment starts.

Published guidelines

Clinical guidelines that name a gene for Krystexxa. They sort people into groups by the gene copies they carry.

CPIC guideline: G6PD. Normal · Indeterminate · Deficient with CNSHA · Variable · Deficient. Strong. 2023.

US FDA: G6PD. Deficient · Variable · Deficient with CNSHA.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

G6PD G6PD A- 202A_376G: Level 3. Toxicity: Hemolysis, Methemoglobinemia. 2.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
