Paromomycin is an aminoglycoside that is barely absorbed from the gut, so taken by mouth it acts on parasites in the intestine: it treats amoebiasis and some tapeworm and Cryptosporidium infections. Given by injection, it treats visceral leishmaniasis (kala-azar), and as a cream, cutaneous leishmaniasis. It is sold as Humatin, among other names.
As for all aminoglycosides, the key gene is MT-RNR1, in the DNA of mitochondria, inherited from the mother. Variants such as m.1555A>G make aminoglycosides bind the mitochondrial ribosome and can cause permanent hearing loss, which matters mainly when paromomycin is injected. CPIC has published a guideline on MT-RNR1 and aminoglycosides, which covers paromomycin.
