About Aminoglycosides

What it is

Aminoglycosides are a class of antibiotics that bind bacterial ribosomes and make them misread genetic instructions. The class includes gentamicin, tobramycin, amikacin, streptomycin, neomycin and plazomicin, used for serious Gram-negative infections, sepsis, some tuberculosis and, inhaled, lung infections in cystic fibrosis. Kidney damage and hearing loss are their main risks. This page collects studies on aminoglycosides as a group.

How genes come into it

The key gene for the whole class is MT-RNR1, in the DNA of mitochondria, which is inherited from the mother. Variants such as m.1555A>G (rs267606617), m.1494C>T and m.1095T>C make the mitochondrial ribosome resemble a bacterial one, so aminoglycosides can cause permanent hearing loss, sometimes after a single dose. CPIC has published a guideline on MT-RNR1 and aminoglycosides. Most consumer DNA chips do not read these mitochondrial positions reliably.

Rated evidence (6 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

MT-RNR1 rs267606617: Level 1A · replicated. Toxicity: Ototoxicity. 31.

MT-RNR1 rs267606619: Level 1A · replicated. Toxicity: Ototoxicity. 8.

MT-RNR1 rs267606618: Level 1A · replicated. Toxicity: Ototoxicity. 3.
