Nilotinib (Tasigna)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Nilotinib

What it is

Nilotinib is a targeted cancer medicine, a tyrosine kinase inhibitor: it blocks BCR-ABL, the abnormal fusion protein that drives chronic myeloid leukaemia (CML). It is used for CML that carries the Philadelphia chromosome, as first treatment or when imatinib has not worked. It is sold as Tasigna.

How genes come into it

Nilotinib blocks the liver enzyme UGT1A1, which clears bilirubin, so bilirubin often rises during treatment. People with the low-activity UGT1A1 version behind Gilbert's syndrome (two copies of *28) are more likely to develop high bilirubin, which the US FDA lists as affecting safety; there is no CPIC or DPWG guideline. Whether the medicine works depends on the BCR-ABL fusion and mutations in the leukaemia cells, which arise in the cancer and are not part of the DNA a person was born with.

Published guidelines

Clinical guidelines that name a gene for Nilotinib. They sort people into groups by the gene copies they carry.

Rated evidence (4 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

UGT1A1 UGT1A1*1, UGT1A1*6, UGT1A1*27, UGT1A1*28: Level 3. Toxicity: Hyperbilirubinemia. 2.

ABCG2 rs2231137: Level 3. Other. 1.

ABCG2 rs41282401: Level 3. Other. 1.

ABCG2 rs1061018: Level 3. Other. 1.

Response (3 findings)

Studies of how well it worked in people carrying a variant.

CYP1A1 rs1048943: Genotypes CT + TT is associated with increased response to nilotinib in people with Leukemia, Myelogenous, Chronic, BCR-ABL Positive as compared to genotype CC.

GSTP1 rs1695: Allele A is associated with increased response to nilotinib in people with Leukemia, Myelogenous, Chronic, BCR-ABL Positive as compared to allele G.

ABL1 rs121913459: Allele T is associated with resistance to nilotinib in people with Leukemia, Myelogenous, Chronic, BCR-ABL Positive as compared to allele C.

Dosing (1 finding)

Studies of how much of the drug people needed or tolerated.

UGT1A1 UGT1A1*1, UGT1A1*6, UGT1A1*27, UGT1A1*28: UGT1A1 *6/*6 + *6/*28 is associated with decreased dose of nilotinib in people with Leukemia, Myelogenous, Chronic, BCR-ABL Positive as compared to UGT1A1 *1/*1 + *1/*6 + *1/*28 + *27/*28.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
