About Nefazodone

What it is

Nefazodone is an antidepressant of the SARI group (serotonin antagonist and reuptake inhibitor): it blocks 5-HT2A serotonin receptors and weakly blocks serotonin reuptake. It was used for depression; because of rare but serious liver injury, it was withdrawn in most countries in the early 2000s and remains available only in a few, as a generic in the US.

How genes come into it

Nefazodone is broken down by CYP3A4, which it also strongly blocks, and its active breakdown product mCPP is cleared by CYP2D6. The few gene studies listing it come from studies of several antidepressants and looked at FKBP5, HTR1B and ABCB1 for response and side effects. The findings are weak, and there is no CPIC or DPWG guideline.
