About Lumefantrine

What it is

Lumefantrine is a long-acting antimalarial that is always combined with artemether (as Coartem or Riamet). Artemether clears most parasites quickly, and lumefantrine, which stays in the blood for days, mops up the rest. It is one of the most used treatments for uncomplicated falciparum malaria worldwide, and is taken with fatty food for good absorption.

How genes come into it

Lumefantrine is broken down mainly by CYP3A4, with CYP3A5 contributing. Studies, many in African patients also taking HIV medicines, looked at CYP3A4, CYP3A5 and CYP2B6 variants, partly because efavirenz, whose levels depend on CYP2B6, speeds up lumefantrine's breakdown. The findings come from a few studies, and there is no CPIC or DPWG guideline.
