Letermovir (Prevymis)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Letermovir

What it is

Letermovir is an antiviral that blocks the terminase complex cytomegalovirus (CMV) needs to package its DNA into new virus particles. It is used to prevent CMV infection and disease after stem cell and kidney transplants, as tablets or by drip. It is sold as Prevymis.

How genes come into it

Letermovir is taken into the liver by the transporters SLCO1B1 and SLCO1B3 and cleared mainly by UGT1A1 and UGT1A3 into bile. Studies have looked at SLCO1B1, UGT1A1 and ABCG2 variants for its blood levels. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (3 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

SLCO1B1 rs4149032: Level 3. Metabolism/PK. 1.

SLCO1B1 rs4149056: Level 3. Metabolism/PK. 1.

UGT1A1 rs4148323: Level 3. Metabolism/PK. 1.

Metabolism (6 findings)

Studies of how quickly the body processes this medication.

SLCO1B1 rs4149032: Genotype TT is associated with decreased exposure to letermovir as compared to genotypes CC + CT.

SLCO1B1 rs4149056: Genotypes CC + CT are associated with increased exposure to letermovir as compared to genotype TT.

UGT1A1 rs4148323: Genotypes AA + AG are associated with increased exposure to letermovir as compared to genotype GG.

ABCG2 rs2231142: Allele T is not associated with exposure to letermovir as compared to allele G.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
