About Iguratimod

What it is

Iguratimod is a disease-modifying antirheumatic drug (DMARD) that suppresses the production of inflammatory signals and antibodies, partly by blocking NF-kappaB. It is used for rheumatoid arthritis in Japan and China, often with methotrexate; it is not approved in Europe or the US. It is sold as Careram and Kolbet.

How genes come into it

Iguratimod is broken down in the liver, partly by CYP2C9 and CYP3A4. The gene studies listed, from China, looked at ABCB1, ABCG2 and NAT2 for response. The findings come from a few studies, and there is no CPIC or DPWG guideline.

ABCG2 rs2231142: Genotypes GT + TT is associated with increased clinical benefit to iguratimod in people with Arthritis, Rheumatoid as compared to genotype GG.

NAT2 rs1495741: Genotypes AG + GG is associated with decreased clinical benefit to iguratimod in people with Arthritis, Rheumatoid as compared to genotype AA.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
