Fenofibrate (Tricor)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Fenofibrate

What it is

Fenofibrate is a fibrate: it activates PPAR-alpha, a switch in liver and muscle cells that speeds up fat burning, which lowers triglycerides and somewhat raises HDL cholesterol. It is used for high triglycerides and mixed blood fat disorders, usually alongside diet. It is sold as Tricor, Lipofen and Lipanthyl, among other names.

How genes come into it

Fenofibrate is cleared mainly by UGT enzymes, not by the CYP enzymes, and no gene has strong evidence for it. Studies have looked at genes of fat transport for how much triglycerides fell, such as APOA5, APOA1, APOC3 and APOA4, at its target PPARA, and at inflammation markers such as CRP. The findings come from single or a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (17 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

APOA5 rs3135506: Level 3. Efficacy: Hypertriglyceridemia. 3.

ABCA1 rs2230806: Level 3. Efficacy: Hypertriglyceridemia. 1.

CRP rs3091244: Level 3. Efficacy: Metabolic Syndrome. 1.

IL6 rs1800795: Level 3. Efficacy: Cardiovascular Disease. 1.

Response (44 findings)

Studies of how well it worked in people carrying a variant.

ABCA1 rs2230806: Genotype CT is associated with increased response to fenofibrate in people with Hypertriglyceridemia as compared to genotype CC.

ABCA1 rs2230806: Genotype TT is associated with increased response to fenofibrate in people with Hypertriglyceridemia as compared to genotype CC.

ABCA1 rs2230808: Genotype CC is associated with increased response to fenofibrate in people with Hypertriglyceridemia as compared to genotype TT.

APOA1 rs2727786: Genotype CG is associated with decreased response to fenofibrate in people with Hypertriglyceridemia as compared to genotype CC.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
