Entacapone (Comtan)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Entacapone

What it is

Entacapone is a COMT inhibitor: it blocks catechol-O-methyltransferase outside the brain, so less levodopa is broken down and more reaches the brain. It is taken with each dose of levodopa in Parkinson's disease to reduce 'wearing-off' between doses, also in the combination Stalevo. It can turn urine orange. It is sold as Comtan.

How genes come into it

Entacapone acts on COMT itself, and the gene study listed looked at the common COMT Val158Met variant (rs4680), which sets how active the enzyme is, for how much entacapone improved levodopa's effect: people with the high-activity version gained more in that study. The evidence is weak, and there is no CPIC or DPWG guideline.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

COMT rs4680: Level 3. Efficacy: Parkinson Disease. 1.

Response (1 finding)

Studies of how well it worked in people carrying a variant.

COMT rs4680: Genotype GG is associated with increased response to entacapone in people with Parkinson Disease as compared to genotype AA.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
