About Dalcetrapib

What it is

Dalcetrapib is an experimental CETP modulator that raises HDL cholesterol by changing how cholesterol is transferred between lipoproteins. Its large dal-OUTCOMES trial did not reduce heart events overall, and it is being studied further only in people with a particular genotype. It is not approved.

How genes come into it

Dalcetrapib is one of the clearest examples of a medicine re-tested by gene. A genome-wide analysis of dal-OUTCOMES found that people with the AA genotype at rs1967309 in ADCY9 had fewer heart events on dalcetrapib, while those with GG had more. A new trial (dal-GenE) tested it only in people with the AA genotype. There is no CPIC or DPWG guideline.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

ADCY9 rs1967309: Level 3. Efficacy, Toxicity: Acute coronary syndrome. 2.

Response (1 finding)

Studies of how well it worked in people carrying a variant.

ADCY9 rs1967309: Genotype AA is associated with increased response to dalcetrapib in people with as compared to genotype GG.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
