Cilostazol (Pletal)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Cilostazol

What it is

Cilostazol is a phosphodiesterase-3 (PDE3) inhibitor that widens arteries and reduces platelet clumping. It is used to increase walking distance in people with intermittent claudication, leg pain from narrowed arteries, and in parts of Asia to prevent further strokes. Its US label carries a boxed warning about heart failure. It is sold as Pletal.

How genes come into it

Cilostazol is broken down mainly by CYP3A4 and CYP2C19, with CYP3A5 contributing. The gene studies listed looked at CYP3A5 and CYP2C19 variants for its blood levels. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (1 rated variant)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

CYP3A5 CYP3A5*1, CYP3A5*3: Level 3. Metabolism/PK. 1.

Metabolism (2 findings)

Studies of how quickly the body processes this medication.

CYP2C19 CYP2C19*1, CYP2C19*2, CYP2C19*3: CYP2C19 *2 + *3 are associated with increased concentrations of cilostazol in healthy individuals as compared to CYP2C19 *1.

CYP3A5 CYP3A5*1, CYP3A5*3: CYP3A5 *1/*3 (assigned as intermediate metabolizer phenotype) is associated with decreased concentrations of cilostazol in healthy individuals as compared to CYP3A5 *3/*3 (assigned as poor metabolizer phenotype) .

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
