Chlorthalidone (Thalitone)

Raw research findings for this medication. Mostly single studies, not curated or confirmed robust.

About Chlorthalidone

What it is

Chlorthalidone is a thiazide-like diuretic: it blocks salt reabsorption in the kidney, so more salt and water leave the body, and over time it relaxes blood vessels. It lasts longer than hydrochlorothiazide and was the diuretic used in the large ALLHAT trial. It is used for high blood pressure and fluid retention. It is sold as Thalitone and Hygroton, among other names.

How genes come into it

Chlorthalidone is cleared mostly unchanged by the kidneys and binds to red blood cells, which gives it its long action. Gene studies on it come mainly from the GenHAT part of ALLHAT and looked at NPPA, AGT, ACE and MMP3 for blood pressure response and heart outcomes. The findings come from a few studies, and there is no CPIC or DPWG guideline.

Rated evidence (7 rated variants)

PharmGKB weighs all studies of a variant and rates how strong the evidence is, from 1A (strongest) to 4.

NPPA rs5065: Level 3. Efficacy: Cardiovascular Disease, Coronary Disease, Stroke. 1.

rs200148: Level 3. Toxicity: Hypertension. 1.

ACE rs4291: Level 3. Efficacy: Hypertension. 1.

MMP3 rs35068180: Level 3. Efficacy: Hypertension. 1.

Study findings and evidence levels: PharmGKB (CC BY-SA 4.0). Guidelines: CPIC and DPWG, as bundled in PharmCAT. Most studies were done mainly in people of European ancestry.
