Carfilzomib is a proteasome inhibitor: it irreversibly blocks the proteasome, the cell's machine for breaking down unwanted proteins, so faulty proteins pile up and myeloma cells, which make huge amounts of antibody protein, die. It is given by drip for relapsed or hard-to-treat multiple myeloma, usually with dexamethasone and other medicines. It is sold as Kyprolis.
Carfilzomib is broken down quickly in the blood by peptidases and epoxide hydrolases, not mainly by CYP enzymes. The few gene studies listed looked at KDR and VEGFA, genes of blood vessel signalling, possibly in relation to heart and blood vessel side effects. The findings come from a few studies, and there is no CPIC or DPWG guideline.
